Photo Release Form

1. Authorization to Use Media

I, the undersigned parent or legal guardian of the above-named child (“Participant”), authorize East Liverpool Youth Academy (“Academy”) to photograph, film, and record the Participant during Academy activities, including but not limited to sports programs, training, competitions, community clean-up projects, and related events.

2. Use of Media

I grant the Academy permission to use, reproduce, publish, and distribute photographs, videos, or audio recordings of the Participant in print materials, social media, websites, and other promotional or educational platforms. These materials may also be shared with community partners and sponsors.

3. Waiver of Rights

I waive any right to inspect or approve the finished product in which the Participant’s likeness appears. I also waive any rights to royalties, compensation, or other claims related to the use of these materials.

4. Release of Liability

I release and hold harmless East Liverpool Youth Academy, its staff, volunteers, representatives, and affiliates from any and all claims, demands, or causes of action connected to the authorized use of these images or recordings.

5. Consent and Certification

I certify that I am the parent or legal guardian of the Participant and have the authority to grant this permission. I have read and understood this release, and I voluntarily agree to its terms.
