Liability Release Form

1. Acknowledgment of Risk

I, the undersigned parent or legal guardian of the above-named minor child (“Participant”), acknowledge and understand that participation in activities organized by East Liverpool Youth Academy (“Academy”) — including but not limited to athletic training, practices, games, competitions, community clean-up projects, and related events — involves inherent risks of personal injury, illness, property damage, or death.

2. Assumption of Risk

On behalf of myself and the Participant, I voluntarily assume all risks, known and unknown, associated with participation in Academy activities, even if arising from the negligence of the Academy or its representatives, and I accept full responsibility for the Participant’s involvement.

3. Release and Waiver

In consideration of the Participant being permitted to participate, I, for myself, the Participant, and our respective heirs, executors, administrators, and assigns, hereby fully release, waive, discharge, and covenant not to sue East Liverpool Youth Academy, its directors, officers, employees, volunteers, agents, affiliates, and sponsors (collectively “Released Parties”) from any and all claims, demands, actions, or causes of action arising out of or relating to participation in Academy activities, i...

4. Indemnification

I further agree to indemnify, defend, and hold harmless the Released Parties from any and all liability, damages, costs, or expenses (including reasonable attorney’s fees) arising from the Participant’s involvement in Academy activities.

5. Medical Authorization

In the event of an emergency, I authorize the Academy and its representatives to secure necessary medical treatment for the Participant. I understand and agree that I am financially responsible for any medical expenses incurred.

6. Severability

If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

7. Acknowledgment and consent

I certify that I am the parent or legal guardian of the Participant and have the authority to execute this Agreement. I have read this document thoroughly, fully understand its terms, and voluntarily sign it with the intent to be legally bound.
